ARC Request Form
Requirement
All members seeking to make any alteration to the exterior and/or common elements of their unit and/or common property that is under the control of the Association, shall first complete this ARC Request Form. Unit owner(s) shall submit this form and all required attachments to management for presentation to the Association’s Board of Directors for their consideration.
Review and Notification
The Association’s Board of Directors shall have thirty days to review all requests and attachments submitted. Upon completion of the Board’s review, written notification of the Board’s decision of approval or denial of the submitted request shall be provided from management to the unit owner(s). All decisions of the Board regarding approval or denial are final, and not subject to appeal.
Required Attachments (email all attachments directly to Bonnie@drires.com)
Proof of contractor’s license number and copies of liability and workers compensation insurance policies;
Copy of written proposal and/or contract to perform work;
Copy of any and all required municipal permits;
Copy of proposed plan and specifications;
Signed attached neighbor(s) affidavit as attached.
By submitting this request form I attest that all information provided within and attached to be true and accurate. I further understand that the Association or its Board of Directors are under no obligation to approve this request. I acknowledge my responsibility to also make application for approval to the River Strand Master Association, as may be required.
